RN position summary

Job Description: RN will initiate and coordinate the hospice care plan, provide skilled and palliative care and coordinate the services provided to patients. The RN works cooperatively with the interdisciplinary team to follow the plan of care for the patients. Performs prescribed medical treatments, including pain management and symptom control, conducts assessments and evaluations, provides education and supportive care to the patient and caregivers.

Core Competencies: Ability to work with dying patients and their families. Possesses a sympathetic attitude towards caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team. Minimum of one-year experience as a RN in Hospice and Palliative Nursing desired.

LVN position summary

Job Description: LVN provides skilled nursing care visits and provides prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the family, or other primary caregiver, and participates in the care plan development and implementation.

Core Competencies: Ability to work with dying patients and their families. Possesses a sympathetic attitude towards caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team. Current CA license, one year experience as an LVN in Hospice and Palliative Nursing.

Hospice Home Health Aide Summary

Job Description: The CHHA provides personal care services to the terminally ill patient and performs related tasks in the patient/family place of residence in accordance with the plan of treatment as assigned by the RN Case Manager. Duties assigned are to increase the level of comfort, and to maintain personal hygiene and a safe, healthy environment for the patient and to provide for temporary respite and relief from total care responsibilities for the primary caregiver.

Core Competencies: Ability to work with dying patients and their families. Possesses a sympathetic attitude towards caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team. Holds a current Hospice Aide certification in the State of CA and demonstrates experience as a hospice aide or completion of an appropriate hospice orientation and training program.
